Personnel Records
Personnel records are of utmost important to the individual employee, his department head, and the University as a whole. Complete records are very helpful in the selection of personnel for future promotions and/or salary increases. Such records include the information about the employees performance on the job, attendance and punctuality, education, training, previous job experiences, length of service and salary history. All these information are highly confidential in nature.
A. Personal Record
Every employee of the college must have in his personal and service records with the Personnel Office all of the following:
1. Latest passport-size photography (two copies)
2. SSS Number
3. BIR-W-4
4. Residence Certificate Number for current year
5. Tax Identification Number
6. Government Licenses (if required for position)
7. Birth Certificate or the Baptismal Certificate of the employee
8. Marriage Certificate, when applicable
9. Xeroxed copy of Transcript of Records, Special Orders, Diplomas, Certificate
B. Service Record
1. Personnel Requisition form
2. Application form
3. Results of Tests
4. Contract of employment or authorization for employment
5. Interview sheet
6. Performance Evaluation Reports
7. Reclassification and/or transfer forms
8. Approved applications for leave
9. Recommendations and censure
10. Resignation/Retirement Papers
